Kidney Stones Patient Medical Case

Female patient, 64 years of age, mentally stable, normal ambulation. Comes to medical consultation complaining of sharp pain all around the lumbar area, local, but sometimes it spreads down to both legs or both inguinal areas. Patient hasn't noticed any external factor to trigger the pain, which it goes partially away after taking paracetamol 750 mgs.
This type of pain has been recurrent for at least 4 times in the last 6 months. She has visited 2 other medical doctors and "none of them have been able to get rid of the pain", the patient told me.
Hyperthermia is occasionally present, especially at night and when the pain is grater. No other symptoms are present.

Patient had G: IV  A: 0  CS: 0  ND: IV. Post-menopause, 13 years, currently without symptoms, not taking hormonal therapy. No surgeries in her medical history, no other chronic diseases either nor physical trauma. There is no history of DB nor CA in the family.

I proceed to perform physical auscultation which reports pain when pressing on lumbar area. Fever is not present at the time of exploration. Pain does not increase when bending. These is all I have to diagnose. The other two doctors diagnosed Lumbalgia and treated as such.

UA reports: epithelial cells ++++, hematuria: micro +, albuminuria ++, leukocytes +++, bacterias +++,
CBC reports increased leucos. Rest of blood tests are normal.
Renal US reports lithos in both kidneys.

CONCLUSION:

Taking some interest in the patient,  in his/her medical case, will always lead us to the right diagnosis and medical treatment.

I have been so busy this summer, with my vegetable garden flourishing (it's giving me broccoli, cauliflower, zucchini, peppers, peas, green beans, tomatoes, celery, you name it!), the cycling season is HOT HOT HOT (I am training like a maniac...HA!) and my own medical practice...my hands are full. So, little time for blogging.
But I needed some work on one of my bicycles today and have had no time to take it to the bike shop for fixing, so I decided to teach myself some bicycle mechanics and came across this fantastic video. It saved me some precious time.
The mechanic on the video knows how to explain it, fast, easy and to the point.

Where is the Outrage?

Reading some financial blogs, I came across this interesting comment. Worth reading.

Where is the outrage? We have become a nation of sheep. I posted the other day about a couple of incidents that happened to me on Saturday at Shipley’s Donuts and PetCo and how so many companies seem to longer care about their customers, other than to get their money. This morning I was discussing this same subject with a business associate and he told me about an incident with his dry cleaners. It seems that he always dropped the clothes off in the morning and picked them up on the way home. The other day he had a late meeting and his wife picked up their cleaning. When Doug got home his wife commented on the cost of the cleaning and it seemed high to Doug. Well it seems that they were over-charged. Doug stopped back at the cleaners and complained. The owner or manager explained that Doug’s wife was charged a higher price because she didn’t “ask” for the special prices. Doug was not offered or given a refund. I asked him if he was going to find a new cleaners and he said he wasn’t, he had been going there a long time. Consumers, voters, taxpayers, etc. have lost the power that they once had, because they no longer use it!

Remember when Coke changed their recipe and a very vocal number of Coke drinkers were outraged and refused to buy the “New Coke”? The outrage was so great and the pressure on the company so strong that Coke went back to their old recipe, calling it Coke Classic. Without passing the cost on to their customers. It’s amazing that such outrage and pressure could be mounted over a soft drink, but not about bad government or a poor economy.

I find it sad that we, as a Nation, seem to placidly accept the bad behavior and incompetence of our leaders. We quietly accept their making themselves elite above us and govern in deference to the true elite. We allow them to do as they please with very little complaint or active response from us; the bail-outs, including TARP and all of the largess given to the incompetent, but “too big too fail” banks and companies. This also includes the squandering of not only trillions of dollars, but of our children’s and grandchildren’s future. It also includes changing the rules to hide the incompetence and failures of the banks and manipulating the data to reassure people about a non-existent recovery. Where is the outrage?

I haven't written any post in the last two days because I have been cycling like crazy. The weather is fantastic, it's sunny, warm and dry. I can finally wear my jerseys without warmers underneath and enjoy the sun rays. I am on my bike in the morning for 3 to 4 hours, come home, change and go to work...on my bike! I do errands on my bike. And, the Tour de France is going on...Also, my vegetable garden is giving me lots of tomatoes, carrots, onions, broccoli, cauliflowers, all kinds of peppers, peas, green breans, corn, you name it. Add to all this, my professional medical practice and feeding my six cats...and there is no time for blogging, extensively.

However, I will be publising posts, but they will be more concise and to the point, they will be more fun, less tedious.

I hope you all do your homework and good luck.

Tour de France Stage 1 Massive Crashes

Stage 1 starts calmly but as kilometers are ridden along the way, multiple and massive crashes occur. Alessandro Petachi has an incredible ability to avoid cycling crashes, so he manages to stay on his bike without falling in the last kilometer massive crash. He crossed the finish line in first place, winning the stage, but didn't help him in the General Classification very much. He is in 38th place with 48 seconds behind "Fabulous" Cancellara.

Alberto Contador VS Lance Armstrong


Due to the popularity of this post (originally published on July 10, 2009) and due to the Tour de France 2010 which will start just few hours from now, I decided to republish this article.

The rivalry between Lance Armstrong and Alberto Contador is interesting and it promises to continue from July 3 to July 25.

A lot of Armstrong and Contador fans will definitely defend their favorite and will support him until the end. But let us not forget that there are many other pro-cyclists who could win this tour. Some experts on cycling are not betting on Armstrong to win, Only time will tell. Lance has been preparing really well to give it a final shot to win an eighth Tour de France crown.

It is true that Lance is older than many others who are favorites to win this tour, but intelligence and experience counts a lot in this strenuous most exciting and difficult competition of the world.

Let's rock and roll, amigos! Let's start the TOUR DE FRANCE 2010!

Today's stage was simply electric, exhilarating, fantastic. Lance Armstrong was excellent and put a good fight. However, I think that he -Lance- either just let Contador go first or Contador took everyone by surprise firing to the end.
But, what was the purpose, to get ahead of Lance? Because let's face it, just at one miserable kilometer away from the finish he -Contador- started sprinting away like it was the big thing. Well, whoopty-do ! He only got ahead of Lance Armstrong by few seconds and he knew he didn't have the chance to win the yellow jersey today, not with Nocentini so ahead of him, and he wasn't a good team player neither.
No, Contador was not a good teammate to Lance. Lance has been really nice and professional, pulling the whole Astana team and with him he brought Contador way up in the General Classification, so I think that Contador was a little ahead of himself.
What was he trying to prove? That he was better than Lance? To me it proved that he is immature and a brat. One pathetic kilometer by surprise, sure I even could have done it myself, if I were a traitor.
Why didn't he try that "phenomenal" escapade 8 or 9 kilometers back? Answer: because he knows he's not good enough to keep up with Lance that far and on that kind of a steep hill. It was all a surprise factor. The terrible thing here is, that you never want to play this card -surprise factor- against YOUR OWN cycling teammates!
Lance Armstrong behaved as a real gentleman. Once "Contadorcito" left, Lance didn't go after him, he let him go and stayed put on his saddle holding Cadel Evans back. Yes, he sat on Evans wheel to allow Alberto build a lead.
Contador is lucky. If Lance would have been to Contador like Hinault was to LeMond, Contador wouldn't have had it so easy.

I admit that Alberto Contador is a fantastic cyclist, but he lacks lots and lots and LOTS of experience. He's just a little brat.
